The Sleep Lab is located below Shelby Medical Associates. For registration please go to Shelby Medical Associates located at 711 N. DeKalb St  (perpendicular to rt 18 at Cleveland Regional Medical Center on the map to the left) and accessible from Schenck St. right across the street from the Social Security building. Parking is available right at the entrance to the Sleep Lab.
